Description of Services

1. Companion – A companion primarily provides sociability and reassurance for a person who lives alone, or during times
of the absence of the primary care person. A companion can also assist with light housekeeping, laundry, menu planning
and meal preparation and providing transportation and assistance for shopping, errands and to medical appointments. A companion does not

assist with personal care.


2. Care Giver – Non-Certified. A Care Giver provides assistance with home management including housekeeping, laundry, menu planning and meal preparation, reminding and assisting with

self-administration of medications and transportation assistance to shopping, errands and medical appointments.

A Care Giver can assist with bathing, personal grooming and hygiene, dressing and undressing. 
 

3. Care Giver – Certified. A Certified Nursing Assistant can provide all of the services as described for Companion and
Care Giver. A Certified Care Giver has

additional training and experience to assist

with personal care such as bathing,
dressing and undressing, mobility, toileting and incontinence care, assistance with feeding, medication administration,
catheter care and skin care.

 

4. Nursing Services – Nursing services are provided by a Registered Nurse. Nursing Services may only be provided to home
care clients whose medical conditions are predictable and stable. A Registered Nurse

will conduct nursing assessments,
monitoring, delegation and/or assignment of specific nursing tasks including diabetic

insulin injections, tube feedings,
wound dressing changes and tracheotomy care. Nursing assessments by the Registered Nurse will only be conducted for those clients requiring or requesting specific nursing tasks.

 

5. Medication Services -Medication services

are offered at three different levels:

Reminding and assisting with
self-administration of medications.

 

Assistance with medication management which would include set-up and monitoring.

 

Medication administration. Medication management and medication administration services are provided under the direction and supervision of a Registered Nurse.

 

Fees for services

 

A minimum of 2 hours per visit is required to obtain services. There is a reduction in fees based on the number of weekly hours of care.

Hourly  10-39 hours per week   

Single   $17.00 per hour          

Couple  $19.00 per hour

 Hourly 40-84 hours per week  

Single   $16.50 per hour

Hourly 85-168 hours per week

Overnight care    14 hours          

Single   $156.00 per night

Additional Charges

$.25 per hour for all hours of care provided

on the weekend

 

$.70 per hour for hourly care provided between the hours of 10 p.m. and 6 a.m.

               (Note: this is calculated into the overnight care charges

 

$ 1.50 per hour additional charge for services provided on Holidays:

  New Year’s Day; Memorial Day; Independence Day (July 4th); Labor Day; Thanksgiving; and Christmas

 

Mileage Charge

$.50 per mile shall be charged when an employee uses their own automobile to run client’s personal errands

 

Travel Charge

There is no charge for travel up to 25 miles one way. There will be an additional charge of  $20.00 per day for travel in excess of 25 miles one way.

 

Cancellation Policy

  A 24 hour cancellation policy is required for services.  Notification of less than 24 hours will result in full charges for the hours scheduled.
